Ellis Primary Care Sites Recognized for Quality
SCHENECTADY – Four Ellis Medicine Primary Care practices have received national recognition for their patient-centered, coordinated model of care.
Ellis Primary Care offices in Clifton Park, Glenville, and Schenectady (both McClellan and Nott Street, Schenectady locations) and have earned Level 3 Patient Centered Medical Home (PCMH) status by the National Committee for Quality Assurance (NCQA). The Level 3 recognition is the highest status achievable and demonstrates that these Ellis Primary Care practices have the systems and processes in place to meet defined, evidence-based standards for delivering high quality care.
“A ‘Medical Home’ is a team approach to providing personalized, comprehensive healthcare for each patient,” explained Dean Limeri, MD, FACP, FAAP, medical director of Primary Care at Ellis Medicine. “Putting patients at the center of their care and encouraging them to play an active role on their care team is critical to achieving better health for patients and our healthcare system.”
A patient-centered medical home is an innovative model of care that focuses on building strong physician-patient relationships through better coordination of care and improved communication. The medical home model aims to provide more personalized, effective and efficient care through improvements to accessibility, communication, tracking of test results, and the integration of care both within the practice and among specialty providers and support services when needed.
