Insurance

We accept most insurances, including Medicare and Medicaid. However, it is your responsibility to be sure that our physicians participate with your insurance and to notify your insurance carrier with the name of your doctor to list as your Primary Care Physician.

Please familiarize yourself with your insurance coverage. Benefits and responsibilities vary. You must bring your insurance card(s) with you to your first visit and any time you get a new card or change insurances. You will be asked to sign a waiver or reschedule your appointment if we cannot verify your insurance coverage with an insurance card.

Some services may not be covered by your insurance. We make every attempt to advise you of this prior to the procedure. In this case, you will be asked to sign a waiver indicating that you are responsible for payment.

Patients Without Insurance

For patients without insurance, please contact Ellis Medicine's Financial Advocates at 518.243.1695 to see if you qualify for free or low-cost health coverage in such programs as Family Health Plus, Child Health Plus or Medicaid. Learn more about our Financial Assistance programs.
